1. Call to schedule an appointment. We have convenient hours before work, during lunch and after work.

2. After you call to schedule an appointment, fill out New Patient Intake Forms (download below) and bring to your first visit. Intake forms are brief paperwork describing you, your health history and your health goals.

3. On your first visit, you will sit down with the Doctor to review your health concerns and medical history.

5. Upon examination, a complete musculo-skeletal examination will be performed. The purpose of a thorough examination is to determine the underlying cause of your problem so that we can provide the best individualized plan to correct your problem and prevent it from returning.
